AND WAS NOT PERMITTED TO CONTINUE AS A PATIENT AND USE MY REGULAR HEALTH INSURANCE. NEVER HEARD FROM HER AGAIN. REFUSES TO FILL OUT FORMS, BECAUSE SHE CLAIMS THAT SHE DOES NOT GET PAID BY ANYONE TO DO SO. VERY UNHELPFUL! |
