I do have concerns with my bill. All information in regards to my name, address and insurance provider was all incorrect. This has caused great difficulty. I would like to know where and how the information provided is retrieved from. Due to the incorrect information on my bill received my primary insurance carrier is declining to settle amount owed. I would like to speak with a person in charge of these situations to clarify.
